Chronic Care Management (CCM)

Programme Provision

Funding is provided so that patients receive the following:

4 free GP practice visits per annum
6 hours nursing time per annum
Structured notes
Extended support from a network of community health teams
A patient held wellness plan about their condition and how to best manage it
Training, feedback and IT support for providers to help manage the patient according to the latest guidelines.
